The Executive Summary Report of a Serious Case Review conducted by the Wrexham Local Safeguarding Children Board (LSCB) has been published. It relates to the case of an 18 day old infant who sustained a head injury which could have been potentially life-threatening as the result of suspected abuse by a care giver in May 2008. The child made a full recovery. The LSCB has a statutory responsibility to review such incidents to identify steps that might be taken to prevent similar incidents occurring in future.
Agencies involved with the child and the child’s family produced reports identifying key aspects of their involvement. These reports contributed information to an overview report which was compiled by an independent author appointed by the Local Safeguarding Children Board. The full report was sent to the Welsh Assembly Government on 25th May 2009. The Executive summary of that report is published today after the conclusion of legal proceedings and police investigations into the case.
The report contained 4 recommendations which have implications for practice. The Local Safeguarding Children Board accepted these recommendations which have all been implemented.
The Serious Case Review concluded that the one area of concern in this case was that, when the child was first examined in the Accident & Emergency Department of the District General Hospital, no consideration was given to the possibility that the injuries could have been caused non-accidentally.
The Serious Case Review identified significant elements of good practice across the Health Service and Children’s Social Care. There was prompt attention by agencies to child protection concerns. The agencies involved in the review have taken steps to communicate the findings of the review to relevant staff.
Wrexham LSCB and member agencies wish to acknowledge that deliberate injury to any child is a deeply saddening event and will continue to work as closely as possible together to keep children safe.
